MOVAO Bari: A Bridge to Walking Again for Patients With Severe Leg Weakness

For patients with profound weakness after critical illness, surgery, prolonged hospitalization, or extended bed rest, walking may be the goal, but standing may not yet be possible.

That creates one of the biggest challenges in physical rehabilitation: How do you progressively strengthen the legs when a patient is too weak to safely exercise against their full body weight?

The Missing Step Between Bed Exercise and Standing

Traditional rehabilitation often progresses from range-of-motion and bedside activity toward sitting, standing, and eventually walking. But patients with severe leg weakness or ICU-acquired weakness may reach a point where basic bed exercises are no longer challenging enough, while standing against full body weight remains too difficult or unsafe.

This is an important population. A 2025 review reported that approximately half of critically ill adults experience ICU-acquired weakness, which can be associated with longer mechanical ventilation, poorer mobility, and impaired physical function.

Recent evidence also supports progressive, individualized rehabilitation for critically ill patients rather than relying solely on passive activity.

That is where gravity-reduced leg strengthening can provide another step in the mobility continuum.

Phase 1: ROM Exercise and Sitting EOB

For a critically ill or profoundly weak patient, rehabilitation may begin while the patient is still dependent on significant medical support, including mechanical ventilation.

Early treatment may include:

  • Range-of-motion (ROM) exercise
  • Bed mobility
  • Sitting at the edge of the bed (EOB)
  • Trunk control
  • Early muscle activation
  • Progressive activity as medically appropriate

Research continues to support early rehabilitation and mobilization for appropriately selected mechanically ventilated ICU patients, with individualized assessment and safety monitoring being essential.

But eventually, the patient needs more than movement. They need resistance to rebuild strength.

Phase 2: Leg Strengthening With MOVAO Bari

Instead of requiring the patient to immediately support 100% of their body weight in standing, MOVAO Bari allows clinicians to use a graded incline to reduce the effects of gravity and progressively challenge the lower extremities.

The patient can perform closed-chain leg strengthening while fully supported, providing a bridge between severe weakness and weight-bearing activity.

As strength improves, the rehabilitation team can progressively increase the challenge based on the patient's abilities and clinical status.

For patients who cannot yet safely stand, the question becomes:

Why wait until a patient is strong enough to stand before beginning progressive leg strengthening?

Phase 3: Standing With a Platform Walker

As lower-extremity strength, activity tolerance, and postural control improve, the patient may progress to supported standing.

For some patients with profound weakness, a platform walker provides greater upper-body support than a traditional rolling walker and can help introduce upright weight bearing and early standing activity.

The goal is progression—not jumping directly from bed exercise to independent walking.

Phase 4: Gait and Endurance Training

Once the patient can safely tolerate standing and begin advancing their legs, rehabilitation can progress toward gait training with a rolling walker, therapist assistance, and increasing walking distance.

At this stage, treatment may focus on functional strength, balance, endurance, gait mechanics, and reducing the amount of assistance required.

The Goal: Independent Walking

The final destination is different for every patient, but for many, it is simple:

Walk again.

The journey may begin with a patient who cannot sit independently, cannot stand, and cannot support their own body weight. Recovery can require many incremental steps.
